The practice provides online services and you are able to manage your appointment bookings, request repeat prescriptions and view your summary medical record online.
If you wish to register for Patient access you will need to come into the surgery with a current photo ID and a proof of address (preferably a current bank statement). Practice staff will then verify your identity face-to-face and a copy of your documents will be taken. We will then activate your account and provide your login details within three working days. You will need to collect this document from the reception desk in person.
Proof of ID - acceptable documents:
Valid drivers licence
Proof of Address - acceptable documents:
Bank Statement - within the last 3 months showing your current address
Drivers licence - must be valid with current address
Your unique login details will be available to collect from reception in 3-5 working days.
